What should I bring with me when I come for a new patient appointment?
- Driver’s license or a valid ID
- Insurance information
- Referral letter (if required)
- New patient paperwork
- Reports, X-rays, MRI’s, CT scans etc. and any other relevant information that was not already sent to us by your referring doctor
- List of current medications and supplements you are taking.
What should I expect my first visit?
During your initial visit, the doctor will take a detailed medical history and perform a neurological examination. He will explain the specific test(s) or procedure(s) you might need. Your therapy options will be discussed and a plan of care will be established. Please arrive 30 minutes before your scheduled appt. time in order to fill out necessary paperwork and HIPPA forms.
What if I have to cancel my appointment?
We ask that you give us a minimum of 24 hours or one business day notice if you need to cancel or reschedule your appointment. Otherwise, you will be assessed a $50 no-show fee, which is not covered by insurance.
What form of payment do you accept?
We accept checks, cash, Visa, and Master Card. Your co-pay is required at the time of service.
How do I obtain prescription refills?
There are a few ways to get your routine medications refilled:
- Call our office and request it. In case if there is no answer please leave a message with your name, date of birth, medication and its dosage, and pharmacy phone number. Please allow at least 1-2 business days to process your request.
- Register with MyChart and request your refill through the portal.
- Ask your pharmacist to submit request electronically.
